New Clinic Signup
Sign up to become a Strive Compounding Pharmacy provider, and begin filling your patient's prescriptions by mail or pickup at one of our convenient pharmacy locations. After your application has been accepted by a member of our team, we will set you up with a login to LifeFile, the portal where you'll submit prescriptions for your patients.
3 Quick Steps to Start Prescribing:
- Fill out the "New Clinic Inquiry Form" located below.
- After you submit it, we will email you an additional form to collect information about specific providers at your clinic who will need to prescribe.
- If you choose the "Bill Office" option below, we will also include the link to a "Clinic Billing Form" in that email.
